CBD may have a less predictable effect on THC than is generally believed.

A new meta-analysis conducted by researchers at the University of Montreal Hospital Center concludes that taking both cannabinoids simultaneously may increase the amount of THC and, in particular, its active metabolite, the 11-OH-THC, present in the blood.

Posted in Neuroscience and Biobehavioral Reviews, the study compiles data from 14 clinical trials involving 341 participants. Its objective was to determine whether the’combination of CBD and THC altered the pharmacokinetics of THC—in other words, the way it is absorbed, metabolized, and eliminated by the body.

However, the results do not mean that CBD consistently makes products stronger. On the contrary, the researchers emphasize the limitations of the available data. But they challenge a widely held belief: that increasing the ratio of CBD to THC is enough to reduce the adverse effects of THC.

An increase in THC exposure, but not necessarily in its peak level

In their analysis, the researchers did not observe a statistically significant increase in the peak THC concentration in the blood, known as Cmax. The overall result was therefore insufficient to conclude that CBD consistently causes a greater increase in blood THC levels. The level of certainty regarding this observation was, in fact, deemed very low.

In contrast, another pharmacokinetic indicator, AUCt—which corresponds to total THC exposure over a given period—was significantly higher when CBD was administered simultaneously.

The ratio of the averages indicates an exposure of approximately 31% (Upper) in the groups that received THC with CBD. This result held up in a sensitivity analysis designed to assess the impact of certain estimated or missing data. However, the authors again rate the certainty of the evidence as very low.

In other words, CBD does not necessarily seem to cause a significantly higher peak concentration of THC, but it may prolong or increase overall exposure to it.

11-OH-THC at the Heart of the Findings

It's mainly on the 11-OH-THC, the main active metabolite of THC, the effects become more pronounced.

The meta-analysis finds a significant increase in both its blood peak and its total exposure when CBD and THC are administered together. For the Cmax of 11-OH-THC, the ratio of the means was 1.468, while it was 1.996 for the AUCt. The researchers assigned a moderate level of certainty to both of these results.

The phenomenon also appears to be linked to the CBD dose. Subgroup analyses show, in particular, greater effects at doses of 100 mg or higher. The meta-regression reveals a dose-response relationship between the amount of CBD administered and the concentrations of 11-OH-THC in the blood.

This observation is particularly interesting for products taken orally. When a product containing THC is ingested, the The liver plays an important role in its metabolism and produces more 11-OH-THC than inhalation does. This metabolite is itself psychoactive and may be as potent as—or even more potent than—THC, even though its effects have been studied much less extensively.

Why CBD Might Alter THC Metabolism

The researchers propose a pharmacokinetic explanation. The CBD may inhibit certain liver enzymes in the family of cytochromes P450, including CYP2C9 and CYP2C19. These enzymes are involved in the metabolism of THC and 11-OH-THC.

By inhibiting their activity, CBD could therefore alter the rate at which these molecules are metabolized and eliminated, thereby promoting their presence in the bloodstream. This mechanism had already been suggested by previous studies and appears consistent with several findings included in the new meta-analysis.

One of the studies included in the review provides a particularly striking example: an oral dose of 640 mg of CBD When combined with 20 mg of THC, it increased plasma concentrations of THC and 11-OH-THC, along with changes in the activity of enzymes involved in the metabolism of certain drugs.

These findings are consistent with a study published in 2024, which had already highlighted a similar phenomenon. In this randomized trial involving 37 volunteers, a high dose of CBD (450 mg) combined with 9 mg of THC was associated with an increase in the psychoactive effects reported by participants, whereas doses of 10 and 30 mg of CBD did not significantly alter the experience.

The new meta-analysis, however, sheds a different light on the matter: by pooling data from 14 clinical trials, it suggests that the interaction could be partly explained by a change in THC pharmacokinetics and a greater accumulation of 11-OH-THC in the blood. The two studies thus agree on one point, though they do not allow us to conclude that CBD systematically enhances the effects of THC: the dose and the context of use appear to be decisive factors, and further research is needed to precisely link variations in blood concentrations to the effects experienced.

No definitive conclusion on the perceived effects

This is probably the most important point to remember: a higher blood concentration alone does not allow us to accurately determine the intensity or duration of the effects experienced.

The authors note that some of the studies using the highest oral doses of CBD also observed more subjective, psychomotor, or cognitive effects in the groups that received both cannabinoids. However, the meta-analysis is not designed to establish a precise link between the observed blood concentrations and behavioral or adverse effects.

The results should therefore be interpreted with caution. The studies analyzed show considerable heterogeneity, with varying doses, formulations, routes of administration, and user profiles. A significant proportion of the studies also presented a high risk of bias.

Another limitation: the trials have primarily examined a single dose. However, many people use the CBD Every Day, which could result in a different pharmacokinetic profile. The researchers are therefore calling for larger clinical trials that more closely mimic real-world conditions of use, particularly the chronic administration of CBD.
