In 2023, Portugal proved to be a hub important aspect of the medical cannabis in Europe, shipping 11 metric tons abroad. In contrast, the domestic market remains challenging, with sales in pharmacies totaling only 17 kilograms. Despite a pioneering medical legalization in 2018, the country is facing a access limited access and a persistent reliance on the black market for patients in need.

No Access to Medical Cannabis in Portugal

Despite initial progress toward legalization, access to medical cannabis remains extremely limited in Portugal. Only one product—dried flower imported from Tilray—was available in 2023, intended for a limited number of patients suffering from specific conditions for which no pharmaceutical alternatives exist. The exorbitant price of €150 per 15-gram package further discouraged local adoption, driving patients toward illicit sources.

Yet the country has become a center for the production and import/export of medical marijuana, particularly to destinations such as Germany, Poland, and Australia, underscore its potential in the global market. In the first half of 2023 alone, Exports exceeded 5.4 metric tons, a promising indicator of the industry's growth trajectory, which already existed in 2021. This success obviously stands in stark contrast to the sluggish domestic sales and the situation facing Portuguese patients.

Somai Pharmaceuticals, a key player since 2019, plans to play a leading role in the field of medical marijuana, although it does not currently operate in Portugal. Founder Michael Sassano anticipates market penetration but acknowledges the difference in scale compared to major players such as Australia and Germany.

Carla Dias, president of the Portuguese Cannabis Observatory, advocates for better access. The recent approval of three new products based on medical cannabis in Portugal is already a sign of progress. In addition, a citizens’ initiative by the European Commission seeks to improve the regulations, promising potential changes at the EU level.
